New Patient Registration
Patients living within the practice area are welcome to register. You must live permanently within our catchment area of the practice to qualify (click here for a map). Our reception staff can confirm your eligibility from your postcode and in this respect please bring along proof of address such as a recent utility bill.
You will also need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team.
Medical treatment is available from the date of registration. Please contact reception for further information.
* Please note. We can only process registrations between 12-6pm (Monday-Friday) as we can be considerably busy in the mornings. On a Saturday between 10am-12.30pm.
If you are wanting to register with us at Soho Square General Practice, you will need to print off the two following documents and bring them in to us at the practice.
